 Voluntary Group Term Life

 

Provided By: United States Life insurance CompanyThe Voluntary Group Term Life Insurance Plan is available for all active full time employees of Member Firms, under the age of 70, working 20 hours or more per week, including spouse under the age of 70, and dependent children. Voluntary Group Term Life Insurance is available on a "stand alone" basis. Medical or Dental coverage is not required to enroll in the Voluntary Group Term Life Insurance Plan. Acceptance is based upon a short-form medical questionnaire.

 

COVERAGE

The employee and spouse may apply for insurance in $10,000 units up to a maximum of $300,000 not to exceed 5 times the employee's basic annual earnings. The spouse can apply and be covered even if the employee does not apply for this coverage. Dependent children can be insured for $2,500 units up to a maximum of $10,000. The employee or spouse must be insured in order for the dependent children to be eligible for coverage. Insurance benefits will terminate at age 70.

The insured's rates will increase as he or she enters the next age bracket on the first of the month following birthday.

*Dependent children are eligible if they are between the ages of 15 days and 25 years. However, children must be attending an accredited college or university on a full-time basis from age 21 to age 25 and be dependent on the employee for support in order to remain eligible for this coverage.

Exclusions

Suicide is excluded during the first two years of coverage.
