● What is Open Enrollment?
-Employers can add, delete or change plans, waiting periods and
contributions for health plans offered to employees.
-All eligible employees can add, delete or change health plan
offered by employer.
● When is Open Enrollment?
-During February and March. Deadline for original applications is February 28, 2010.
-Effective April 1, 2010.
● What is the website: www.qaia.com You can get all forms from this site.
-If you prefer we can send you an Administrative Packet that has all form
templates and enrollment instructions- Just request one.
● How to move to a different plan with the same carrier?
-Complete a “Transfer List” for each company.
-If the change is from a PPO to HMO or visa-versa, a new application must be completed.
● How can you change Carriers?
-Only plans chosen on PA (Participation Agreement) are available.
-Complete a new application for medical or a CHANGE form for ancillary products.
●How do we add/delete dependents to existing plans?
- Complete a CHANGE form, marking identifying sections and the changes desired.
● What is legally required minimum employer contribution?
-50% of lowest plan, per carrier, offered by employer.
● When can employees come onto the program?
-At completion of company’s waiting period for full time employees
and foronly 30 daysthereafter. After that they are INELIGIBLE!
-If they lose other coverage elsewhere
-Qualifying Event-Life status change: marriage, divorce, loss of employment, birth, etc.
● When will my ID cards come? Medical, Vision & Chiro/Acup
-Usually within 30 days of the effective date. Call us if you haven’t received them.
-Metlife Dental & Metlife Life do not send cards. Tell your provider you have
‘Metlife’ and the subscriber’s Social Security number.
● How do I change doctors?
-The HealthNet system will not allow us to change your doctor. You must do it
for needed info.
-Only you can change your doctor through Kaiser. Call Member Services at 800-464-4000.
● What do I do with billing errors?
-To the best of your ability, write a description of the problem, fax it and
a copy of the bill with notations where the problem seems to be to:
