                                2010 Health Plan Renewal Summary
 
The following is a summary of the changes to watch for this year as you review your options.
ALL RENEWAL PAPERWORK IS DUE BY FEBRUARY 28TH, 2010.
 
Term Life Insurance – MetLife
o        Small rate changes for both Basic and Optional (Voluntary) term life products
o        Both are Portable for 30 days after terminated employment
 
Dental –  1 Carriers : MetLife   No Rate changes
o        MetLife- Increased IN-Network preventative from 80% to 100%; Implants now covered
o         3 Tiers: 1-5; 6-19; 20+
o        Ortho coverage for 6+ enrolled EEs- Old Premier Access Ortho coverage grandfathered in.
 
Voluntary Multi-life disability income – Met Life- Same Great Carrier!!
o        Non-cancelable; guaranteed renewable policy
o        Up to 15% discounts available to the employees of all member firms
 
Chiropractic / Acupuncture - American Specialty health Plans of California, Inc.
o        Chiropractic and Chiropractic & Acupuncture – No rate or benefit changes
 
Vision – 2 Carriers to Choose from : Blue view and vsp
 
      VSP (Vision Service Plan)
o        Same Vision Plan benefits with addition of polycarbonate lenses for children under 18
o        Discounts even after benefits are completely used
Blue view vision (Division of Anthem Blue Cross)
o        Same Vision Plan Benefits with a 30% discount compared to 2006 rates
o        Additional Retail Providers – Lens Crafters, Pearle Vision, Sears Optical
o         
Medical - Required Participation in a medical plan is now 100% for Groups 1-2;  75% for Groups 3+
 
Kaiser
o        Three Rate Tiers based on group size and carrier underwriting
o        New Kaiser Accounts tier based on size or direct Kaiser tier rate(if previously with Kaiser)
o        Existing Kaiser Accounts – Tier status based on experience rating by Kaiser underwriting
o        One New Plan –   $40/2000
o        $0/1500 is changed to 0/2000; discontinued $0/2200 & HRA plans
o        All plans except K15 & K20 are available to groups of 1&2 enrolled
o        Benefit structure for HSA plans have separate deductibles & OOP maximums
 
HealthNet
○    New HN accounts tier based on size only 1-5 groups Tier 1; Groups 6+ minimum Tier I-V based
      on Individual and/or Employer health questionnaires determined by HN.
o        Existing Health Net Accounts – Tier status based on experience rating by Health Net underwriting
o        New HSA Plan –  Value HSA 4500: Deductible matches OOP maximum, pays 100%
o        HMO 40, HSA Value 4500 plans are available to groups of 1&2 enrolled
o        New PPO Value 30
o        New Health Net Accounts can apply for discounted rate tier by submitting health statements
(6-9 enrolling) or an Employer Statement (10+ enrolling) to Health Net underwriting
○    75% for groups 1-9 when offered with Kaiser
o        Existing & New groups >10: HealthNet Participation Updates (if offered with Kaiser)
o        60% if only Health Net HMOs and HSAs are offered alongside Kaiser
o        70% if any Health Net PPO (non-HSA) Plans are offered alongside Kaiser
o        There is NO minimum participation requirement if only 1 Health Net HSA (no HMO or PPO plans) is offered alongside Kaiser AND a minimum of 6 eligible employees enroll with Health Net.
o        Options Plans; HSAs 1500, 2000 & 3500; HMO Value 20, HMO ST. 30 & PPO ST. 40
are being discontinued. Unless otherwise chosen, automatic migration to nearest benefit structure will change enrollment.
