An employer may elect only one complementary rider (i.e., High Option or Low Option)
All employees covered under the Builders' Exchange health plan must be enrolled in the rider - this is not a voluntary product
Employees not covered under the Builders' Exchange health plan are not eligible for the rider.

These riders are available only to employees that have coverage under one of the Exchange medical health plans:
(Health Net, Kaiser) - this is not a stand alone product

Flexible Choice	Over 1,500 ASHP participating Chiropractors and over 600 participating Acupuncturists
	Over 1,000 ancillary radiology and clinical laboratory providers
	ASHP provider chosen at point of service
	No pre-designation form required
	Change ASHP provider at any time
Easy to Use	Co-payments based on option selected (shown below)
	No claim forms
	No deductibles
Comprehensive	Initial and subsequent examinations (annual limits apply based on plan elected)
	Office visits and adjustments or acupuncture treatment (annual limits apply)
	Adjunctive therapy (annual limits apply)
	$50 annual chiropractic appliance benefit
	Diagnostic services, including radiology and clinical laboratory services
	ASHP providers obtain authorization for all services based on clinical necessity
	(Other services are not covered)
