These riders are available only to employees that have coverage under one of the Exchange medical health plans:
(Health Net, Kaiser) - this is not a stand alone product

An employer may elect only one complementary rider (i.e., High Option or Low Option)
All employees covered under the Builders' Exchange health plan must be enrolled in the rider - this is not a voluntary product
Employees not covered under the Builders' Exchange health plan are not eligible for the rider.

	Over 1,000 ancillary radiology and clinical laboratory providers

	No claim forms
	No deductibles
Comprehensive	Initial and subsequent examinations (annual limits apply based on plan elected)
	Office visits and adjustments or acupuncture treatment (annual limits apply)
	Adjunctive therapy (annual limits apply)
	$50 annual chiropractic appliance benefit
	Diagnostic services, including radiology and clinical laboratory services
	ASHP providers obtain authorization for all services based on clinical necessity
	(Other services are not covered)
